Who may qualify

You may qualify if all of these are true: 

• You have no dental insurance

• Your adjusted family net income is under $90,000

• You are a Canadian resident for tax purposes

• You filed your tax return for the last year and have your Notice of Assessment

 

If you have dental coverage through a government social program, you may still be able to apply. For more information on Eligibility, visit CDCP's Eligibility Requirements Page

 

 

How to apply

1. Sign in to My Service Canada Account

2. Find Canadian Dental Care Plan on your dashboard

3. Click Apply and follow the steps

If you cannot apply online, you can apply on canada.ca or call Service Canada at 1.833.537.4342. 

 

 

What you need to apply

 

Have this ready for each person you are applying for: 

• Social Insurance Number, if available for children

• Full name and date of birth

• Home and mailing address

• Details of any dental coverage through government social programs

• Proof you filed taxes for last year and your Notice of Assessment

 

 

What does CDCP cover?

CDCP may help cover common services like exams, x-rays, cleanings, fillings, root canals, dentures, and extractions. Some services may need approval first. 

 

 

What you may have to pay?

​

CDCP does not always mean free care. Your share depends on your adjusted family net income. 

• Under $70,000: no copay

• From $70,000 to $79,999: you may pay 40%

• From $80,000 to $89,999: you may pay 60%

 

Note: Your dentist may charge fees that are higher than the CDCP fee guide. If that happens, you may need to pay the difference. Always ask us for an estimate before treatment.

• Tell us you are a CDCP patient when you book

• Bring your CDCP member information, if you have it

• We will review your plan and explain any costs before we start
